Evergreen Crossing – Central City Concern

Evergreen Crossing is home to Central City Concern’s Recuperative Care Program, or RCP, which provides medical respite care, a critical service that provides a safe and supportive environment for people recently discharged from hospitals. Participants are referred directly from local hospitals, health plans and outpatient providers, and they benefit from the service’s continuing and proactive support to effectively recover. The program improves health outcomes for high-risk patients, efficiency of participating hospitals and helps avoid discharging patients back to the street. In addition, the new Evergreen Crossing Primary Care Clinic will soon open to treat clients in RCP.
